I've had some strange behaviour with playlists on my iPod classic, similar to what you describe.  Sometimes they're there, sometimes the ipod just spits out the request to scroll through.  Do you have the Blue & Me adapter for your car?  Nothing much worked until I got one of those.  Now, I tend to queue up what i want to listen to before I start my journey, I find the menus too distracting to use when I'm driving and as I said they still sometimes let me down.
